Medical Record Release Authorization Form
We set aside time each day for care of children with acute illnesses or accidents. In cases of illness, when possible, we ask that you try to call early in the day to enable us to more efficiently organize our appointment schedule. We will strive to run the office as efficiently as we can, recognizing that an occasional emergency may interfere with this goal. We do ask that you try to be on time to help us minimize your waiting period and decrease the likelihood of spreading infection in our reception area.
If you need to cancel an appointment, please do so at the earliest opportunity to allow us to free that time slot for others. If we have advance knowledge of your medical provider being delayed, we will try to notify you to either postpone or reschedule your appointment, so as not to waste your valuable time.
We see newborns at various local hospitals. If you would like more information or a list of hospitals please call us. If a child needs to be hospitalized, we use Children’s Mercy Hospital or Children's Mercy South, which are staffed with medical providers that exclusively care for children. We feel that children receive the best possible care in these facilities.
You are encouraged to see whichever medical provider you prefer. We will try to keep you with that same medical provider as much as the scheduling allows. At times, any of our medical providers may work at other locations to see patients. During office hours your child’s medical record will be readily available to all medical providers who care for him or her. After hours, you may need to familiarize the medical provider taking the call with the child’s history as he or she won't have ready access to the medical record.
Please recognize that many “routine” illnesses are often handled a bit differently by each individual medical provider where a variety of diagnostic and treatment approaches are equally good. We hope you will not become confused or upset should one medical provider treat your child a bit differently than you are accustomed for similar complaints.
We are concerned with providing your child and you with prompt, professional, and courteous service in all the aspects of the office. We encourage you to let us know if you feel these qualities are lacking in any respect (the doctor, nurse practitioner, a consultant, the front office, the nurses, or the answering service). We can only improve on deficiencies when we know they exist.
Urgent Care Clinic Visits
Patients will be seen by whichever medical provider is available first.
We do not charge for telephone consultation during office hours and we can always be reached by calling the clinic’s phone number, 816-554-6520. During office hours your call will be directed to the phone nurse, the receptionist for appointment or the insurance clerk. If the phone nurse is unable to answer your medical questions, she will discuss the case with the medical provider and call you back. If necessary, the medical provider may call you back over the lunch hour or at the end of the day. We prefer to provide medical advice through phone calls instead of email. We do not correspond by email at this time.
After Hours Emergency Phone Calls and Messages
While we may stress our availability by phone, 24 hours a day, we would ask that you defer your non-emergent questions until office hours. We hope to reserve “off-hours” calls for emergencies only. There is a $15.00 charge for these calls to offset the charge of our answering service. Please take call blocker off your phone so that we can return your phone call. If you find it necessary to call after the office has closed, please leave your name, your child’s name and age, your medical provider’s name, and your telephone number with the answering service and we will return your call. If you do not hear back from us in a reasonable length of time, please phone again. If we feel the child needs to be seen before the next scheduled office hours, we will recommend where to take him/her. Occasionally you might not feel you have the time or opportunity to call us before seeking emergency care. Please be informed as to the requirements of your insurance company when going to the emergency room or requesting referrals. The answering service phone number is 913-338-8280.
Calling for Antibiotics
We DO NOT call out antibiotics over the phone, except on rare occasions where the medical provider deems it necessary.
